(Wisdomatics ) Health Industry News, does surveys primarily for health care providers, although I know of them from a healthcare PAYER survey I did last week (how did I get signed up for it...I have no idea, nor know if they have a seperate payer/patient survey panel). I'm supposed to get a $10 Amazon GC.
